How do I make a referral?

Please read the ‘Projects’ section to identify the projects that we currently deliver – under each project description you will find a referral form.

Who can refer?

Families can refer themselves.
Social Workers.
Health Visitors
Support workers
Army Welfare Services.
Midwives.
Community psychiatric nurses.
Teachers and ESCos.
Children’s centre staff.
Probation officers.
Playgroup workers.
GPs.
Other health workers.

How to refer a family/young person

Ask the family’s permission to make a referral.

Talk to the family about our Home-Start services and how we can help (you can either print out the support provided page from this website or we can send you information leaflets).
Parents can also self refer by completing and sending in a completed self-referral form, sending an email or calling the office.
Anyone can complete a referral form found here and forward it to our office – please call for a password if sending by email.

What happens after the referral?

Each referral will be considered by the Scheme co-ordinators at the weekly Allocation Meeting to identify if the referral can be accepted. The referrer will be notified if the referral has been allocated an initial visit time and date. Once accepted the local Co-ordinator will visit the family to further clarify the family’s/child’s needs and the support that Home-Start could offer.
